Unit 10 Assignment

The nurse narrator, child and council are composites. Policy details are described in general terms.
One Child
A two-year-old on my case management caseload had a blood lead level below the reference value at her first test. At her second, six months later, it had risen above it. When I visited, her mother told me the landlord had sanded the window frames in April to repaint them, with no plastic sheeting and no cleanup beyond a broom. The dust settled on the floor and windowsills where she played every day.
What would have prevented it? Not better teaching from me; her mother cleaned carefully. Not faster screening; we found it. What would have prevented it was a rule that the apartment be inspected for lead hazards before it was rented, and that repairs be done safely.
The Policy
The proposal: rental units built before 1978 would be inspected for lead hazards as a condition of registration with the city, with repairs required before a certificate is issued. Rochester, New York adopted a similar ordinance in 2005, and evaluations found high compliance and a decline in children's lead levels in the years that followed, though the authors caution that other changes during the same period also contributed (Korfmacher et al., 2012). I cannot claim the ordinance alone caused Rochester's decline, but it is the strongest local model we have.
The ordinance would be enforced by the housing department. It would cost landlords an inspection fee and the price of repairs, which is the main source of opposition.
Programs That Already Help
Medicaid requires blood lead testing at 12 and 24 months, which gives the health department a way to find exposed children. The city could also apply for federal lead hazard reduction grants to help landlords pay for repairs, which would soften the cost argument.
Will It Pass?
Kingdon describes policy change as happening when three streams meet: a recognized problem, a workable policy and favorable politics (Kingdon, 2011). The problem stream has local data from this term: testing coverage, elevated results, housing age. The policy stream has a tested model. The politics stream is the weak one: the council member for Foundry Row has not taken a position, and the landlord association opposes new fees.
Why This Is Nursing Work
Provision 9 of the ANA Code of Ethics places the shaping of health policy, and the pursuit of justice through it, among the profession's duties (American Nurses Association, 2015). I used to think policy was for other people. The child on my caseload made it mine.
What Nurses Can Do to Move the Streams
The politics stream is where nurses have the most to offer. The council member has heard from landlords; she has not heard from the people who visit the apartments. A nurse can bring what no landlord association can: a child's story, told with permission and without a name, and a map of where elevated results cluster. The coalition built in Unit 9 gives that testimony more weight, because a pastor, a school nurse and a health center manager speaking together are harder to dismiss than one voice.
Nurses can also help the policy stream by answering the cost objection before it is raised. If the city applies for federal lead hazard reduction grants, landlords in Foundry Row could receive help paying for repairs, and the ordinance becomes a requirement with support rather than a fee without it. I have asked the health department's grants coordinator whether the city has applied before, and if not, what an application would require from the housing department and from nurses who could supply the data.
What Changed in Me
Before this term, when I read a patient's address, I read it as a place to send the discharge papers. Now I read it as a set of conditions: the age of the building, the distance to the pharmacy, the trucks on the street. The term changed the belief that health happens mostly in clinics. It happens mostly in homes, streets and rental agreements, and a nurse who only works in the clinic sees the results without seeing the causes. That realization is uncomfortable, because it means the work I was trained for is necessary but not enough.
My next step is concrete: I have signed up to speak for three minutes at the council's public hearing on housing in [month], with the neighborhood's testing data and, with her mother's permission, the story of one child without her name.

How this NU450 Unit 10 example is structured
The reflection moves from one child to a law and back to the writer. The opening case gives the proposal a harm to answer. The policy section describes what the ordinance would require, who enforces it and what it costs landlords, and cites evaluations of the Rochester law with a caution about attributing all of that city's decline to one policy. Kingdon's framework organizes the feasibility discussion: the problem stream has local data from the term's assessment, the policy stream has a tested model, and the politics stream depends on a council member and a landlord association not yet persuaded. The ANA Code of Ethics, provision 9, grounds the claim that shaping policy is part of nursing. The final section is reflective, naming one belief the term changed and one step the writer will take: speaking at a public hearing.

NU450 Unit 10 questions, answered
Which policy framework suits a Unit 10 reflection?
Kingdon's multiple streams model is common because it explains why good proposals often wait for the right moment, which suits a feasibility discussion. Some courses use a policy process model or the levels of government instead. Apply the chosen model to your proposal rather than summarizing it, and cite the edition or source your course assigns.
Does the reflection need to propose a real law?
It helps to propose something specific, and modeling it on an existing law from another place gives you evidence to cite. The proposal does not have to be under consideration where you live. Describe what it would require and who would enforce it, and say honestly what is uncertain about its effects in your community.
How much of the paper should be personal reflection?
It depends on the prompt: some split policy and reflection evenly, and others weight one side. The sample gives roughly a third to reflection, naming a changed belief and a concrete next step. Reflection written in general terms about loving public health earns little; a specific shift in how you practice, stated plainly, earns the most.
